What are the Personal Doctor Specialties with which I can register?
> Beneficiaries up to the age of 15 can register with a PD specialised in Paediatrics.
 
> Beneficiaries who have attained the age of 15 and up to the age of 65 can register with PDs:
  • Specialised in Paediatrics,
  • Specialised in General Medicine,
  • With special training in General Medicine,
  • Who hold a certificate certifying their right to practice General Medicine,
  • Specialised in Pathology, and
  • Who have attended special training

Where can I find the list of PDs who are contracted with the GHS and their specialties?
Information about the PDs who are contracted with the GHS can be found on the HIO Website and the Beneficiary Portal.
How can I register in a PD List and what is the procedure?
> Via the Beneficiary Portal:
1.      Select the PD of your choice and proceed to submit a registration request in their beneficiaries list
  • If you are already enrolled as a GHS Beneficiary via the Beneficiary Portal: you may submit a registration request to a PD list by starting the procedure from the Management of Registration Requests section.
  • If you are not enrolled as a GHS Beneficiary: you may submit your registration request to a PD list during your enrolment procedure as a GHS Beneficiary.
2.      Upon the submission of the above request, the PD shall receive a notification and he/she has the option to reply whether:
  • He/she accepts the request and therefore you are registered in his/her beneficiaries list, or
  • He/she asks you to visit them before registering you in his/her beneficiaries list.  In this case you are not deemed registered in his/her list until you visit the PD.
In both cases above you will be notified by e-mail or letter by post (in case you have not given an e-mail address).
Your registration request to a PD is valid for 7 working days.  In case the PD does not reply within 7 days your request is cancelled and you are not deemed registered in his/her beneficiaries list.
Upon the submission of a request to be registered with a PD, a place is saved on the PD’s list until the PD replies or until the request is cancelled (whichever occurs first).
In order to complete your registration in a PD beneficiaries list you must visit the PD:
  • To confirm your personal information – identify the beneficiary (this is only done once during the first contact of the beneficiary with the GHS), and
  • To sign the form “Mutual Acceptance for Registration in a Personal Doctor Beneficiaries List”
  • By visiting the PD: the PD shall register you in his/her beneficiaries list and you will be notified (by e-mail or letter by post in case you have not given an e-mail address), that you are registered in the beneficiaries list of the said PD.  During your visit with the PD, the PD (or his/her secretary) shall:
o   Confirm your personal information – beneficiary identification  (this is only done once during the first contact of the beneficiary with the GHS), and
o   Sign the form “Mutual Acceptance for Registration in a Personal Doctor Beneficiaries List”,
in order to complete your registration in the PD’s beneficiaries list.
 
If you were already registered in a PD List, upon your registration in the beneficiaries list of a different PD you are removed from the beneficiaries list of the first PD who is informed accordingly.
What happens in case I have submitted a registration request to a PD beneficiaries list via the Beneficiary Portal but in the meantime I changed my mind and I wish to be registered in a different PD’s list?
In case you have submitted a request to be registered with a PD and the reply from the PD is pending you may:
  • Submit a new registration request to a PD via the Beneficiary Portal.  Upon the submission of the new request, the previous request is automatically cancelled.  Only one registration request to a PD list may be valid at any time.
o   This procedure can only be done in case you are already registered with a PD and are in the process of changing your PD (it is not the first time you are registering with a PD)
or
  • Visit the PD with whom you wish to register.  The registration procedure in the beneficiaries list will be completed by the PD.  Upon your registration in the beneficiaries list of the PD you have visited, the previous request is automatically cancelled.
o   This procedure can be done whether it is the first time you are registering with a PD or you are already registered with a PD and are in the process of changing your PD.
Can I submit a registration request to a PD on behalf of another beneficiary?
Beneficiaries who have an account in the Beneficiary Portal may submit a registration request to a PD beneficiaries list on behalf of:
  • Their child up to the age of 18 (dependant)
  • Another person for whom they have submitted a GHS enrolment request through their account in the Beneficiary Portal.
What is the form “Mutual Acceptance for Registration in a Personal Doctor Beneficiaries List”?
The form “Mutual Acceptance for Registration in a Personal Doctor Beneficiaries List” is a written confirmation that:
  • The beneficiary wishes and accepts to be registered in the Beneficiaries List of the said PD, and
  • The PD confirms that he/she accepts the registration of the said beneficiary in his/her Beneficiaries List.
The “Mutual Acceptance for Registration in a Personal Doctor Beneficiaries List” form must be signed between the beneficiary and the PD every time a beneficiary is registered in the beneficiaries list of a PD (changes his/her PD).
One of the two signed original forms remains with the PD and the other is given to the beneficiary.  The HIO may request from any PD to present the signed forms of all the beneficiaries who are registered in his/her list.
Can the PD remove me from his/her beneficiaries list?
The PD has the right to remove a beneficiary who is registered in their beneficiaries list.  In such case, you will be informed accordingly and asked to be registered in the beneficiaries list of another PD within 15 calendar days as of the date of the initiation of the removal procedure.
You remain in the beneficiaries list of the said PD and you may receive services by the PD until you are registered in the beneficiaries list of another PD.
In case you are not registered in the beneficiaries list of another PD within 15 calendar days, you will be automatically registered in the beneficiaries list of another PD via the automatic distribution procedure of the Information Technology system and you will receive a relevant notice of the name of the PD.
Can I change my PD?
You can change your PD after the lapse of 6 months as of your date of registration in the beneficiaries list of your PD (general rule).
 
However, the general rule does not apply in the following cases:
  • Beneficiaries up to the age of 2 may change their PD for children without any restriction
  • Beneficiaries between the ages of 16 and 18 who are registered with a PD for children may register with a PD for adults before the lapse of 6 months
  • In case the HIO terminates the PD’s contract or the PD terminates his/her cooperation with a specific legal person (e.g. company) through which he/she offered healthcare services within the GHS framework
  • After the registration of a beneficiary with a PD through the automatic distribution procedure of the Information Technology system.
What is the automatic distribution procedure?
In case a beneficiary is asked to change his/her PD for any reason and this procedure is not completed within the predetermined time (of 15 calendar days) then the beneficiary is automatically registered in a PD list that is selected according to an algorithm.
After the activation of the automatic distribution procedure, the beneficiary is informed about the name of the PD with whom he/she has been registered and the registration request to a PD (if any) is cancelled.  It is noted that the beneficiary may change his/her PD before the lapse of 6 months as of the date of his/her registration in the PD beneficiaries list where he/she was registered via the automatic distribution procedure.
The automatic distribution procedure is activated in the following cases:
  • A beneficiary has been removed by a PD and has not registered with another PD within the set deadline.
  • A beneficiary has attained the age of 18 and has not register with a PD for adults.
  • A beneficiary has not register with another PD within the set deadline after the termination of the PD’s contract with the HIO.
  • A beneficiary has not registered with another PD within the set deadline after the termination of the cooperation of the PD with a specific legal person (e.g. company) through which he/she offered healthcare services within the GHS framework.
Can I remain registered with a PD for children after I attain the age of 18?
In case a beneficiary is close to the age of 18 and is registered in a beneficiaries list of a PD for children then his/her guardian will receive a reminder that the beneficiary must register with a PD for adults.
If a beneficiary after the attainment of the age of 18 is still registered with a PD for children then he/she will be automatically registered with a PD for adults via the automatic distribution procedure and he/she will receive a relevant notification about the name of the PD.
As a PD, which beneficiaries can I register in my beneficiaries list?
  • PDs specialised in Paediatrics can register beneficiaries up to the age of 18.
  • PDs specialised in General Medicine can register beneficiaries who have attained the age of 15.
  • PDs specialised in General Medicine can register beneficiaries who have attained the age of 15.
  • PDs with special training in General Medicine can register beneficiaries who have attained the age of 15.
  • PDs who hold a certificate certifying their right to practice General Medicine, can register beneficiaries who have attained the age of 15.
  • PDs specialised in Pathology can register beneficiaries who have attained the age of 15.
  • PDs who have attended a special training program can register beneficiaries who have attained the age of 15.
  • PDs specialised in Geriatrics can register beneficiaries who have attained the age of 65.
As a PD, how do I register beneficiaries in my beneficiaries list?
You register beneficiaries in your beneficiaries list by following one of the procedures below:
 
a.  You can register a beneficiary via the Provider Portal upon the beneficiary’s visit to your office
o   The beneficiary’s registration process can be done by you or your support staff (secretary) provided that you have stated this and have given them access to the Provider Portal
o   It is also noted that in the case a beneficiary is added to the beneficiaries list of a PD by the PD himself, the beneficiary shall receive a relevant notification immediately.
b.  A beneficiary can register on his own via the Beneficiary Portal
o   A beneficiary chooses you via the Beneficiary Portal and submits a registration request to your beneficiaries list.  Upon the submission of the request by the beneficiary you shall receive a relevant notification and  asked to reply whether:
c.  You accept the request and therefore the beneficiary is added to your beneficiaries list, or
d.  You ask the beneficiary to visit you before you register them on your beneficiaries list.  In this case, the beneficiary is not registered in your beneficiaries list and the place in the list is no longer saved.
o   It is noted that the registration request to a PD list submitted by a beneficiary is valid for 7 calendar[US1]  days.  In case you do not reply within that deadline the request expires.
o   In addition, a beneficiary has the right, after submitting a registration request to a PD list and the request is pending (i.e. it is still valid) to submit again a registration request to another PD.  In this case, the previous registration request is cancelled and only the registration request that was submitted last is valid.
 
What do I do if I do not wish to register a beneficiary in my list?
There are two cases regarding the rejection of a registration in a personal doctor list:
 
1.      In case a beneficiary has requested to be registered in a personal doctor list, the personal doctor may refuse to register the beneficiary but in that case the beneficiary has the right to lodge a complaint to the HIO.  Specific decisions determine the details of the investigation of a complaint lodged by a beneficiary.
2.      In case the HIO assigns the provision of healthcare services of a beneficiary to a personal doctor who has a contract with the HIO via the automatic distribution procedure, the personal doctor is obliged to register the beneficiary in the following cases:
  • A beneficiary has been removed by a PD and has not register with another PD within the set deadline
  • A beneficiary has attained the age of 18 and has not register with a PD for adults
  • A beneficiary has not register with another PD within the set deadline after the termination of a PD’s contract with the HIO
  • A beneficiary has not register with another PD within the set deadline after the termination of a PD’s cooperation with a specific legal person (e.g. company) through which he/she offered healthcare services within the GHS framework. 
In all cases, a PD has the right to request the removal of a beneficiary from his/her list provided that the removal request is free from any discrimination.
Which procedures can be performed by the support staff in the Provider Portal?
PDs may give access to the Provider Portal to their support staff to support their work.  Access is given via the PD’s account in the Provider Portal.  After the PD completes the “Add Support Staff” procedure, the support staff shall have access to the Provider Portal with different passwords (user name and password) than the ones the PD has.
The support staff can perform the following:
  • Beneficiary enrolment in the GHS Beneficiary Record
  • Beneficiary registration in the PD beneficiaries list
  • Beneficiary identification
  • Print out the form  “Mutual Acceptance for Registration in a Personal Doctor Beneficiaries List”
  • Beneficiary removal from the PD beneficiaries list
  • Set up a visit
What is a beneficiary identification
Upon the first contact of a beneficiary with the GHS, i.e. during the first visit of a beneficiary with a PD or OS, the doctor confirms that the personal information stated by the beneficiary upon their enrolment in the GHS are the same with that written on their identification document (ID Card or Alien Registration Certificate (ARC)).
Where can I find/see the registration requests that have been submitted by beneficiaries for my list?
When a beneficiary submits a registration request to your beneficiaries list, you will receive a notification in the Inbox of your account in the Provider Portal.  However, you can see all the pending registration requests by beneficiaries in the section Personal Doctor – Beneficiaries List – Registration Requests in the Provider Portal.
How many Beneficiaries can I register in my beneficiaries list?
The maximum number of beneficiaries that can be included in your beneficiaries list is 2500.  This number is calculated by the sum of the Registered Beneficiaries and the pending Registration Requests.
 
An exception to the above threshold is the case where the siblings of beneficiaries already registered with a PD for children wish to be registered with the same PD for children while the beneficiaries list of the said PD has already reached the 2500 threshold.  In this case the PD prints a relevant form from the Provider Portal that is filled in and signed by the PD and the beneficiary’s guardian and sent to the HIO by the PD. 
As a PD, can I remove a beneficiary from my beneficiaries list?
In case that for some reason you wish to remove a beneficiary from your beneficiaries list you may do this via the section Personal Doctor – Beneficiaries List.
During the removal procedure of the beneficiary you must record the reason for which you wish to remove them from your list.  The removal reason is not communicated to the beneficiary.
Upon the request for the removal of a beneficiary, the said beneficiary is informed of the fact and asked to register with another PD.
The beneficiary’s removal is activated within 15 calendar days at the latest as of the initiation of the procedure or until the beneficiary is registered in the list of another PD (if this occurs earlier).  Up until the activation of the removal of the beneficiary, the beneficiary remains in your list and may receive PD services from you.
